All students in any Milford I.S.D athletic program must have the proper forms signed and on file at MISD. Please download the forms below and return a signed copy to your coach.

All students entering the 6th, 9th, or 11th grade and all new students need to have the Physical Evaluation Form  completed and turned in prior to participating in any athletic events.  All other students need to check with Coach Crumpton to determine if this form is needed.
